Scholarship awards are available based on financial need, but those seeking scholarships must fax or mail paperwork rather than use on-line registration. All others have the choice of signing up on-lineorprinting forms and sending them by mail or fax and must pay at the time of enrollment to reserve a spot. Tuitions are refundable until June 15 minus a $50 per week administration fee.

MVP Basketball Camp Scholarship Award Application
MVP scholarship awards are made based on financial need or on the basis of a family member serving in the armed forces, and are also conditional on whether the child(ren) make an effort to do their best in the classroom. Follow the instruction on this form and return it with the enrollment application. Please review our scholarship guidelines prior to deciding how many weeks you wish to apply for. Most grants are only for one complete week of camp.
MVP Basketball Camp Health and Release Form
MVP is driven to maintain a healthy and safe environment for all campers. Accordingly we require the completion of this form, which includes standard medical information provided by the parent/guardian, emergency and physician phone numbers, insurance information, and MVP's Code of Conduct Policy, along with a standard photography release. Campers will not be allowed to participate without a completed form.
